After discussion and consideration, a motion was made by Dr. Bertch-Mumaw and 
seconded by Dr. Wright, fo accept the Investigative Committee’s recommendation 
and dismiss this issue with no violation. Motion passed unanimously. 


2. 14-79, In Re: Gurushabd Khalsa, D.V.M. 

Dr. Khalsa was present with attorney, David Stoll, and made statements. After 
discussion and consideration, a motion was made by Dr. Bertch-Mumaw and 
seconded by Ms. Mumford, to disagree with the Investigative Committee's 
recommendation and dismiss this issue with no violation. Motion passed unanimously. 


3. 14-81, In Re: Otto Stanisiaw Jr., D.V.M. 

Attorney, David Stoll, was present to monitor. After discussion and consideration, a 
motion was made by Dr. Christensen and seconded by Dr. Berich-Mumaw, to accept 
the Investigative Committee’s recommendation and dismiss this issue with no

Attorney, David Stoll, was present to monitor. Ms. Donna Ferrante made a statement 
on behalf of Ms. Cynthia Ferrante. After discussion and consideration, a motion was 
made by Dr. Christensen and seconded by Dr. Bertch-Mumaw, to accept the 
investigative Committee's recommendation and dismiss this issue with no violation. 
Motion passed unanimously. 


5. 14-83, In Re: Elena Beis, D.V.M. 

Attorney, David Stoll, was present to monitor. Ms. Donna Ferrante made a statement 
on behalf of Ms. Cynthia Ferrante. After discussion and consideration, a motion was 
made by Dr. Wright and seconded by Dr. Bertch-Mumaw, to accept the Investigative 
Committee's recommendation and dismiss this issue with no violation. Motion passed 
unanimously. 


6. 14-84, In Re: Lisa Schnier, D.V.M. 
Dr. Schnier was present and made statements. Ms. Carrie Smith was present and 
made statements. After discussion and consideration, a motion was made by Dr. 
Serbin and seconded by Mr. Loughead, to accept the Investigative Committee’s 
recommendation and dismiss this issue with no violation. Motion passed, 7 ayes, 2 
nays (Ms. Young and Dr. Hatfield). 


7. 14-87, In Re: William Dean, D.V.M. 

After discussion and consideration, a motion was made by Ms. Gullett and seconded 
by Dr. Hatfield, to accept the Investigative Committee's recommendation and dismiss 
this issue with no violation. Motion passed unanimously. 


8. 14-88, In Re: Maggie Weston, D.V.M. 
Attorney, David Stoll, was present to monitor. After discussion and consideration, a 
motion was made by Ms. Mumford and seconded by Dr. Berfch-Mumaw, to accept 
the Investigative Committee's recommendation and dismiss this issue with no 
violation. Motion passed unanimously. 


9. 14-89, In Re: Deborah Chapman, D.V.M. 

After discussion and consideration, a motion was made by Dr. Hatfield and seconded 
by Dr. Bertch-Mumaw, to accept the Investigative Committee's recommendation and 
dismiss this issue with no violation. Motion passed unanimously. 


Further discussion took place and the Board directed staff to contact, consulting 
veterinarian, Dr. Petcu, to discuss his submission fo the Board in response to the case.

Dr. Anderson was present with attorney, David Stoll, and made statements and answered

502 (L) (3), (4), (5), (6), (7), (8), (9), (10), and (11) as indicated in the premise inspection

seconded by Dr. Serbin to find Dr. Muhoho in violation of: 
ARS. § 32-2232 (21) as it relates to R3-11-502: 
¢ (H) (3) for failure to record the animal's heart rate and respiration rate in the medical 
record at a minimum of every 15 minutes while anesthesia is being administered; and 
¢ (1) (7) (b) for failure to record in the medical record the amount of fluids that was 
administered to the dog on March 13, 2014.

¢ For failure to monitor the dog's urine function and production throughout the day on 
March 17, 2014; 
e For failure to monitor the dog's urine specific gravity throughout the day on March 
17, 2014; and 
e For failure to document the dog's condition throughout the day on March 17, 2014.

Bertch-Mumaw to find Dr. Wanick in violation of: 
ARS. § 32-2232 (21) as it relates to R3-11-502: 

¢ (H) (2) for failure to document in the medical record an examination of the animal 

within six hours before anesthesia is administered or surgery is performed, including

the animal's temperature, heart rate, respiration rate, diagnosis and general 
condition; and 
e (H) (3) for failure to document in the medical record the animal's heart rate and 
respiration rate immediately after giving the animal a general anesthetic and

administered.

violation of A.R.S. § 32-2232 (21) as it relates to A.A.C. R3-11-502 (L): 

*(2) failure to record the dog's weight in the medical record January 21st through 

January 28th of 2014; 

« (4) failure to record examination findings of the dog during hospitalization; 

*(7) (a) and (b) failure to record the concentration and amount of Gentamycin used on 

January 20, 2014; 

*(7) (b) failure to record the amount of fluids administered to the dog; and 

*(7) (c) failure to record the frequency of Ampicillin administered to the dog.

negligence; causing unnecessary suffering or death due to: 

*Continuing Gentamycin to a dog with known kidney issues resulting in renal failure and 

concurrent death; 

*Inadequate fluid administration to a dog with a serious condition; 

e Lack of adequate pain management resulting in the animal suffering; 

«Not performing a urinalysis; and 

*Sending the dog home when in renal failure.

motion was made by Dr. Hatfield and seconded by Dr. Serbin to find Dr. Tascon in violation 
of A.R.S. § 32-2232 (21) as it relates to A.A.C. R3-11-502 (L): 

*(2) failure to record the dog's weight in the medical record January 25! and January 

26! of 2014; 

(4) failure to record examination findings of the dog during hospitalization; and 

*(7) (b) failure to record the amount of fluids administered to the dog.

professional and scientific knowledge for: 
*Not ensuring the dog was receiving an appropriate amount of fluids for dog's 
condition; 
*Not communicating the severity of the dog’s condition and prognosis to the pet owner 
when he was overseeing the care of the dog over the weekend in question; 
*Not ensuring the dog was receiving proper pain medication given the dog’s history 
that a surgical procedure was performed on January 20!,

by Dr. Hatfield to find Dr. Teskey in violation of A.R.S. § 32-2232 (12) as It relates to A.A.C, R3- 
11-501 (1) failure to provide professionally acceptable procedures for not testing the horse’s 
blood until five (5) days after collecting the sample. Roll call: Dr. Serbin, nay; Mr. Loughead,

seconded by Dr. Hatfield to issue Dr. Teskey a Letter of Concern with respect to

communicating to his boarders regarding who will be providing veterinary care for their

violations that are routine entries into a medical record and do not affect fhe diagnosis or 
care of the animal - failure to document the address and phone number of the animal

. ADMINISTRATIVE REVIEW OF PREVIOUS ACTIONS 


1.11-99: In Re: Audie Wood, D.V.M. Board appearance in compliance with Board Order. 
Dr. Wood was present in compliance with her Board Order, made statements and 
answered questions. It was brought to the Board's attention that Dr. Wood's random urine 
screen showed a dilute creatinine level and specific gravity that was in the acceptable 
range by .0004; out of nineteen screens she has completed, five have indicated abnormal 
levels for these two indicators. Dr. Wood indicated she drinks a lot of water, which may 
impact these two indicators. 


2.13-33: In Re: Greg Heltman, D.V.M. Board may review, discuss and take action on 
Practice Monitor Reports. 
Tabled for a future Board meeting. 


3. C-14-12: In Re: Joe Robinson. Board may review and discuss injunction status. 
Tabled for a future Board meeting. 


4,14-27; In Re: Monica Bunge, D.V. M. Board may review, discuss and take action on 
request to lift Suspension status and allow Dr. Bunge to return to work part-time. 

Dr. Bunge was present telephonically, made statements and answered questions. After 
discussion and consideration, a motion was made by Dr. Serbin and seconded by Dr. 
Bertch-Mumaw to table this matter for a future Board meeting when Dr. Bunge has an 
employment offer with details and has relayed her current status to the potential 
responsible veterinarian. The Board will consider an emergency Board meeting if necessary. 
Motion passed, 8 ayes, 1 absent (Ms. Young). 


5. 14-64: In Re: Robert Burkhardt, C.V.T. Board may review, discuss and take action on 
substance abuse evaluation.

After discussion and consideration, a motion was made by Ms. Mumford and seconded by 
Dr. Bertch-Mumaw to deny certification for CVT applicants who failed the VINE and/or 
State exam. Motion passed 7 ayes, 2 absent (Ms. Young and Ms. Gullett). 


4,Board may review, discuss and take action on C-14-13, In Re: Thlago Moraes, unlicensed 
practitioner. 

After discussion and consideration, a motion was made by Dr. Christensen and seconded 
by Dr. Hatfield to issue Mr. Moraes a Cease and Desist for the practice of veterinary

narrated a picture presentation of the premise inspection that had occurred. After 
discussion and consideration, a motion was made by Dr. Serbin and seconded by Dr. 
Berfch-Mumaw to deny Catalina Animal Hospital's premise application pursuant to: 

« ARS. § 32-2274 (A) (2), (3) as outlined in the Board staff memo, (4), (5) and (6); and

Bertch-Mumaw to uphold the Board's 2007 decision designating underserved Arizona 
communities for the purpose of the WICHE program. These are Apache, Graham, Greenlee,
